Well since they are "sexual" artists, they usually are aware of the risks of their profession, so they do get protected, while the most innocent kawaii people are usually naive enough to think they won't ever get anything wrong, and it happens. People who work with disarming bombs are sure to wear protection in case the bomb actually explode or do you think they'd be dumb enough to work at full risk?
Because of that "groups of risks", such as prostitutes and so have long been banned on medicine attention, focusing rather on "risk activities" instead. It doesn't matter what your job is, a nun, or whatever, if you don't know how to use protrction, your at risk.
