It's indeed a shadow, white colour reflects a lot of light, and because of that you only see the detail of the wall in the zone in shadow caused by Ayu's head, if you change the levels in photoshop you can see the wall behind:
It's the same in this picture, the door reflects almost all of the flash light and you only see the detail in the shadow:
The picture is indeed heavily retouched, but they haven't erased her arm nor pasted her under that thing
