Ah, yeah. Like Toniayu suggested, you should write your name in Katakana for the top field (in the section regarding your shipping address).

You can probably just use this site since it's not really important haha
In the second field below that one, you can type your name in English as usual.
And for the field where it asks what name you would like to use to be printed on the album, you can only use upper-case English letters.
