I think it's really the past 3 or so years that it has not looked good. Her TV appearances are generally 1-2 times per year these days, and it always seems like she gets some filler right before appearing. I'm sure this is driven by feeling like she's going to be judged, but it always looks like it hasn't had enough time to settle and she just looks puffy and weird, and more importantly - not like herself. I say this vs. when I saw her in 2015 & 2016 TA tours, very close, and she looked STUNNING. She doesn't need fillers, or at least not overboard like she does before TV appearances. I just wish someone would advise her to time it a little better.
Edit: And with regards to having work done, I also really feel like it has only/mostly been fillers (and of course her teeth, years ago). She has always looked drastically different based on hair and make-up, throughout the full extent of her career. Then she goes and does some candid make-up-free shot and always looks like the same girl.
