She also said a few years back, after she "recovered" from her Eustachian tube problem that she took vocal lessons and she had to change her singing techniques to match her earring dysfunction. It could be why her voice sounds more "raw" nowadays. Plus, she said she tries to challenge herself to sing more with "her soul" rather than just her vocals.
Touring non stop probably didn't help the cause, but honestly she sounds much better than 2011-2013 after her hiatus... She can sing her old songs in concerts as well as on CD, so I would say it's more a technique thing rather than damaged vocals.
If you watch her recent Yuki no Hana lives for instance, she can hit her usual notes but there's more vibrato, as if she was trying to add more "substance" to her interpretation.
Her most recent performance of Glamorous Sky (CDTV) sounded exactly like her past ones whereas in 2011-2012 it was painful to listen to.
So yeah, a little bit of everything, but to me it's more to do with the technique she uses to sing.
