I don't think she's crying in all the off-key parts.. imo @_@; I mean, she's off-key at the same parts in the song (at the beginning and ending of the chorus for example) and she doesn't cry all the time.. but w/e

There's been nothing said that I can recall that said Ayu didn't want to have surgery. What HAS been said is that there was no way to treat what she has.

I'm quite sure people were talking about that Ayu didnt want surgery before, until it was last year and she wanted to have it (or got recommanded to do it?) and then she hard that it was too late. And then she told her friends and family about the ear problem.
Oh and I think I also read on the forum that she can still hear a bit but that there's a loud ringing in the ear. I forgot the disease's name but it was something like tit~something. @_@ I'm quite sure I read that all on the forum, because I can't imagine making it up myself lol.
I dont think the shock of hearing that you cant be cured anymore would stop her from singing properly that much, imo.
