1. is not likely at all. Because if so, she would have written so clearly. So 2. or 3. And I can't guess further more.
Btw, she held a lot of concerts after Dome Tour, too, especially in the year 2002, as you know. Now it has become clear that she had a problem in her left ear, I can't help thinking such a lot of concerts might be bad to her ear.
