"Everyone" have those 3:

Physical gender: Male or Female
Psychological gender: Male or Female
Sexual orientation: straight or gay

You can have any combination of those 3, so, even if Hide for example changed her sex to become a girl, she can still be into girls... Her case is not album how she feels about others, but how she feels about herself. The prejudice name on this case would be "transphobia".

It's not considered to be an mental illness or something cuz only stuff that hurt the person themselves and people around them can be considered such (that's why homossexuality can't be considered an illness either).

And neutral pronouns are useful cuz not everyone is either male or female, the most rare it is, you have a lot of people who are in between, this is called intersex. On this cases, usually the person stick to the role of one of the 2 genders, but it's difficult to apply one gender since childhood to those people because it's not uncommon for them to grow up and discover they identify with the other gender.
